Weight Management Services and Diabetes Prevention Programme

Did you know that we can refer patients to digital weight management services and the national diabetes prevention programme?

In order for a patient to be referred to these services they need to meet a set criterion, please see below.

◼️ Weight management services – patients who have a BMI of 30+ or a BMI of 27.5+ for patients from Black, Asian or minority ethnic groups.

◼️ National Diabetes Prevention Programme (NDPP) Type 2 – patients who are pre-diabetic and have a raised BMI.

If you would like to be referred or are unsure whether you meet the criteria, please fill out an administrative e-consult form, and we will review your request.
