Proxy Access is where someone is given access to another person’s medical record. For example:
NHS England recommend that practices adopt a very cautious approach to allowing parents’ access to their child’s medical record once the child reaches the age of 11. Children vary in the age at which they are able to make an independent and informed decision about who should have access to their record.
Rothschild House Group has decided to adopt the most practical approach and withdraw parents’ access as soon as the child reaches 11 unless there are exceptional circumstances. A young person may decide at a point, once they are mature enough to act autonomously, to request access themselves.
From 11-16, a parent with proxy access will be able to manage certain elements of the young person’s record, such as demographic data, and make appointments and order repeat prescriptions, but they will not be able to see the young person’s past appointments or clinical record, although they would still be able to see the current repeat prescription record.
It is within our care to monitor and record any concerns, and we will remove access to the account if deemed appropriate and in the best interest of the patient. This will be reviewed annually.
Patients aged 16 or above are assumed to have the capacity to consent unless there is an indication that they are not. Young patients between the ages of 11 and 16 who are judged as having capacity to consent by their GP may also consent to give proxy access to someone else.
If you wish to apply for proxy user access, please fill out the correct form below and hand it in to the surgery.
