Health and Wellbeing Coach

Our Health and Wellbeing Coach, Claire, offers a series of 45-minute personalised sessions to equip you with the confidence and skills to take control of your health. Please see below for more information on her role and how it supports our patients:

  • Support and motivate patients with positive lifestyle change to develop the knowledge, skills and confidence to become active participants in their own care.
  • Non-judgmental approach that supports the person to self-identify existing issues and encourages practice prevention of new and existing illnesses, such as Pre-Diabetes, Type 2 Diabetes, High Cholesterol, and Hypertension, as well as others.
  • Weight management, health eating, exercise, sleep and low-level stress management.
  • Patients can book an appointment by contacting our Patient Services Team or they can be booked in by a Healthcare Professional. Claire works across all of our sites.
