Social Prescriber

We have Social Prescribers who work across all five sites, they are non-clinical practitioners.

• Our GPs can refer patients to a Social Prescriber when they feel a patient would benefit from support in signposting to services to improve their health and wellbeing.
• Their aim is to support patients in a holistic way, to connect them with their local community and to give them more choice and control of their lives. They can also signpost to local clubs/activities, Social Care Department and Mental Health Services.
