If you are an adult (over 18) with a learning disability you may like to have a Purple Folder. The Purple Folder provides health information about you. You can use it at all your appointments with health professionals for example doctors, dentists and pharmacists.
This will help them to work with you to give you the care and treatment that you agree is right for you. Your yearly heath check plan can be placed in your Purple Folder too.
If you are someone who goes out and about on your own, without support, you can also obtain a Purple Card. This has some basic information about you, your health and how health professionals can help you.
